Do note that for premium payment using MediSave, the policyholder (payer) must be related to the life assured in one of the following ways:
- Spouse
- Child
- Parent
- Grandchild
If you wish for other family members to make payment for your GREAT SupremeHealth plan, please seek approval from CPF Board.
Do provide evidence of the approval from CPF Board when you submit your request.
How to change payer for your GREAT SupremeHealth plan
1. Download documents
To change policy payer for GREAT SupremeHealth, we'll require you to complete and submit the Change of payer for GREAT SupremeHealth plan form.
2. Include supporting documents
Ensure you include the approval from CPF board if you wish for other family members to make payment for your GREAT SupremeHealth plan.
3. Submission
