Attend a medical examination

There may be occasions where you are required to go for a medical examination prior to purchasing an insurance plan.

If a medical examination is necessary, your Financial Representative will make the arrangements for the medical appointment and accompany you for the visit.

Your Financial Representative will provide the medical examination form to the doctor conducting the medical examination. Alternatively, the clinic can obtain the form from this page.

The doctor conducting the medical examination will submit the completed medical examination form directly to us.

If the medical examination that has been requested by us is conducted by a clinic on our panel, the cost will be absorbed by us.

If the medical examination is conducted by a clinic that is not on our panel, you will need to bear the difference between the cost of a medical examination done at our panel clinic and the one done at a non-panel clinic.
