Home Based Child Vaccination and Developmental Screening Programme (“Campaign”) Terms and Conditions

1. The home based child vaccination services and home based child developmental screening programme (“Programme”) is organised by The Great Eastern Life Assurance Company Limited (“Great Eastern”) in partnership with Haus Medical Pte Ltd (“Haus Medical”). The Programme is available to all individuals regardless of whether they are policyholders of Great Eastern’s insurance products or not, subject to the conditions stated within. All individuals using the Programme are referred to as the “Users”.

2. The home based child vaccination service is offered to children or wards, aged between 2 months to 5 years:

    (a) who are Singapore citizens with NRIC, free of charge; and

    (b) who are non-Singapore citizens with FIN, at a preferred rate with the cost to be borne by the parent or guardian (as the case may be).

    The home based child developmental screening services are offered to children or wards, aged between 2 months to 18 months:

    (a) who are Singapore citizens with NRIC, free of charge; and

    (b) who are non-Singapore citizens with FIN, at a preferred rate with the cost to be borne by the parent or guardian (as the case may be).

3. Types of vaccination to be offered for the child home based vaccination service will be in accordance with those stipulated in the Ministry of Health’s (MOH) National Childhood Immunisation Schedule, and MOH’s Vaccination and Childhood Developmental Screening must be available and in force at the time of the User receiving the child home based vaccination service. The Programme will be performed within Singapore only and are subject to Haus Medical continuing to operate Community Health Assist Scheme GP clinics in Singapore at the time of the User is participating in the Programme and Haus Medical is not having undergone any event rendering it incapable or unable, at its reasonable discretion, of continuing to provide the Programme.

4. Where the Services involve any costs to be borne by the User, such costs shall be paid directly to Haus Medical Registration for a vaccination appointment under the Programme shall commence on 18th of January 2024 using Haus Medical’s registration site for the Programme at hausmedical.sg/child-vaccination (“Registration Link”). Any amendment to the registration period may be effected by The Great Eastern Life Assurance Co. Ltd. (“Great Eastern”) through updating these terms and conditions and/or indicating the same through the Registration Link as it deems fit in its absolute discretion. For the avoidance of doubt, users of child home based vaccination services can only register for the first vaccination or vaccination and childhood developmental screening required for the next relevant milestone.

5. Each registration through the Registration Link must indicate the email address or mobile number of the User’s Great Eastern Financial Representative to proceed with registration. Submissions without email address or mobile number will not be successfully registered.

6. Parents/Guardian who are Users must ensure that they and their child/ward(s)’s registered particulars are true, complete and accurate in all respects, and that Great Eastern and/or Haus Medical may rely on the information provided in the registration form to contact them in connection with this Programme. Great Eastern shall not be liable for any incomplete, lost, late or inaccurate entries for any reasons whatsoever or problems or technical malfunction of any telephone network or lines, computer online systems, servers, providers or technical problems, or any combination thereof.

7. Great Eastern reserves the right to vary or supplement these terms and conditions, or to withdraw the Programme, at any time in its absolute discretion and without any prior notice nor liability to any party. By participating in this Programme, Users and parents/guardians of Users who are children/wards hereby agree to all the terms and conditions as stated within, including all variations or supplements to these terms and conditions (if any).

8. In the event of any inconsistency between these terms and conditions and any advertising, publicity, brochure, marketing or other materials relating to or in connection with the Programme, these terms and conditions shall prevail.

9. Great Eastern makes no representation as to the quality or performance of the services offered in the Programme and the service experience provided by Haus Medical. By participating in the Programme, the User acknowledges and accepts that their participation in the Programme and usage of any of the services and/or any other services that they sign up for directly with Haus Medical shall be subject to the terms and conditions of Haus Medical, and any issues arising from the services provided by Haus Medical will be addressed by Haus Medical.

10. To the fullest extent permitted by law, Great Eastern and its related corporations (collectively, the “Companies”) shall not be liable for or be responsible in respect of any expenses, losses, costs, damages, liabilities or other consequences of whatsoever nature (collectively “Losses”) suffered or incurred directly or indirectly by each User for the Programme, howsoever caused or arising and without limiting the generality of the foregoing, whether by reason of or on account of any act or omission whether negligent or otherwise on the part of the Companies or their respective employees or agents or representatives, even if the User or parent/guardian of the User (as the case may be) have been advised of the possibility of such Losses.

11. By participating in the Programme, the User agrees to release and holds the Companies harmless from any and all liability whatsoever for any injuries, losses or damages of any kind to any person, including their child/ward, or property arising from or in connection with, either directly or indirectly registering for the Programme and using any services which are provided by Haus Medical.

12. Great Eastern’s decision on all matters relating to the Programme is final and binding. Great Eastern will not be obliged to provide the reason(s) for its decision.

13. By participating in the Programme, each User agrees and consents to Great Eastern and its related corporations (collectively, the “Companies”) as well as their respective representatives, agents, authorised service providers and relevant third parties (the Companies and all the other foregoing parties, collectively, “Great Eastern Persons”), collecting, using, disclosing and/or sharing amongst themselves each User’s personal data, for purposes reasonably required by the Companies to administer the Programme and such other purposes as described in Great Eastern’s Privacy Statement which is accessible from Great Eastern’s website, which each User and parent/guardian of a child/ward who is a User confirms he/she has read and understood.

14. In addition, by participating in the Programme, each User agrees that relevant information including personal data that is collected and used by Haus Medical and its related companies and affiliates in connection with this Programme, can be disclosed to Great Eastern, to be collected, used and disclosed for the purpose of Great Eastern’s representatives and agents informing Users of their or their child/ward’s appointments for the Services, updates on the Great Eastern products and other ancillary purposes.

15. The terms and conditions of the Programme shall be governed by and interpreted in accordance with Singapore law. The courts of Singapore shall have exclusive jurisdiction over any disputes arising from the terms and conditions, including the validity and enforceability thereof.
