For official proof and verification, we would require you to submit the following documents for the type of claim you are making.

1. Instruction Page
2. Junior Living Assurance Claimant’s Statement 
3. All available Laboratory and Test results (as specified in the Doctor’s Statement for the type of claim which you are claiming for)
4. Authorisation Letter (if claimant is authorizing another GEL agent to handle the claim (including collection of cheque) 
5. Doctors Statement :
   • Bone Marrow Transplant
   • Brain Surgery 
   • Glomerulonephritis with Nephrotic Syndome
   • Insulin-dependent Diabetes Mellitus
   • Intellectual Impairment due to Accident or Sickness
   • Kawasaki Disease with Heart Complications
   • Leukaemia
   • Rheumatic Fever with Valvular Impairment
   • Severe Asthma
   • Severe Juvenile Rheumatoid Arthritis
   • Major Cancers
   • Aplastic Anaemia
   • Encephalitis
   • Hydrocephalus
   • Kidney Failure
   • Major Organ Transplant
   • Severe Epilepsy
   • Systematic Lupus Erythematosus with Lupus Nephritis

Once you have downloaded and completed the relevant claim forms, please submit the claim to us together with the required documents for assessment.
