How to reinstate your policy
Follow the steps below to reinstate your Dependants' Protection Scheme (DPS).
1. Download document
The forms needed are different depending on your policy status.
Policy status | Reinstatement period | Form required |
Lapsed | Within 120 days from renewal date | Dependants' Protection Scheme application for reinstatement/top-up form |
Terminated due to opt out | Within 30 days from termination letter date | Opt-back form on the overleaf of the termination letter |
After 30 days from termination letter date - within 120 days from renewal date | Dependants' Protection Scheme application for reinstatement/top-up form | |
After 120 days from renewal date | Dependants' Protection Scheme Proposal form |
2. Make payment
To reinstate your lapsed or terminated policy, the following payment methods are accepted:
- Internet banking
- AXS
- CPF
- Cheque
- Cash or Nets at our Customer Service Centres
3. Submission
Submit the completed form to us via email.
