Discharge Voucher For Withdrawal/Surrender For Investment-Linked Plans (PSF33)           

The Policy Owner of the above mentioned Policy had agreed to take the following action with regard to the Investment-linked policy, on the understanding that: - 

  1. the effective price will be as at the date and cut-off time Great Eastern Life Assurance (Malaysia) Berhad receives this fully completed application and requirements.         
  2. the expected outcomes of the sustainability test will be provided within 14 days after the fully completed application is received.                 
  3. the policy's Total Investment Value (“TIV”)'s may not be sufficient to sustain your policy up  to full contractual term/maturity date and may lapse when the TIV is depleted.
