Discharge Voucher For Withdrawal/Surrender For Investment-Linked Plans (PSF33)
The Policy Owner of the above mentioned Policy had agreed to take the following action with regard to the Investment-linked policy, on the understanding that: -
- the effective price will be as at the date and cut-off time Great Eastern Life Assurance (Malaysia) Berhad receives this fully completed application and requirements.
- the expected outcomes of the sustainability test will be provided within 14 days after the fully completed application is received.
- the policy's Total Investment Value (“TIV”)'s may not be sufficient to sustain your policy up to full contractual term/maturity date and may lapse when the TIV is depleted.
